Qualité Des Soins Aux Urgences Médicales Du Centre Hospitalo-universitaires De Tlemcen: Place De La Coordination Entre Les Personnels Intra Et Interservices.

Résumé : On entend par « urgence » le besoin exprimé d’une prise en charge diagnostique et thérapeutique rapide, non programmée, entraînant la consultation ou l’hospitalisation dans un établissement de soins. En Algérie, les consultations et hospitalisations en urgence représentent une part importante des activités de soins du secteur public (Kessai et al, 2013). Abrouk et al, (2006) témoigne de dysfonctionnements particulièrement organisationnels. Le service de médecine du travail du Centre hospitalo-universitaire de Tlemcen (CHUT) recense des demandes récurrentes de changement de services par les médecins généralistes. Ce dernier incite la directrice de l’hôpital à intervenir. Celle-ci dernière s’engage par une demande d’intervention ergonomique. La prise en charge d’un patient aux urgences relève de l’intervention coordonnée de plusieurs acteurs médicaux et para médicaux appartenant au service des urgences et aux services internes de l’hôpital. Le malade usager de ce service est souvent accompagné par ses proches parents. Ils deviennent acteurs de cette chaine Jackson (1998) cité par Querelle et Thibault, 2007 ; Gaudart, Weill-Fassina (1999) cité par Caroly et Weill Fassina, 2004 ; Flageul-Caroly (2001) ; Weill-Fassina et Pastré (2004) cité par Caroly et Weill-Fassina, 2007. Le patient et ses interactions avec les acteurs de la chaine de soins au service des urgences médico-chirurgicales (UMC) est l’objet de notre intervention ergonomique. La qualité des soins est l’enjeux visé. Des entretiens avec les personnels concernés ainsi qu’avec les patients et/ou leurs parents (Caroly, 2010) complétés par Une analyse du contexte et des observations ont été réalisés. La coordination interne et avec les services internes de l’hôpital est la demande analysée. Cette coordination est relative à La substitution du parent de malade aux ressources humaines du service des UMC, d’une part et au Manque de compétences opérationnelles entre les différents acteurs de la chaine de soins d’autre part (Caroly, 2010 ; Caroly et al, 2006 ; Weill-Fassina, 2004). L’organisation d’une unité d’accueil et de tri des patients ainsi que la coordination entre les UMC et les services internes de l’hôpital selon des collectifs efficaces, ont été préconisées. The patient and his interactions with the actors of the chain of care in the medical and surgical emergency department (UMC) are the subject of our ergonomic intervention. The quality of care is the target. Interviews with the personnel concerned as well as with the patients and /or their parents Caroly, (2010) supplemented by an analysis of the context and observations were carried out. The internal coordination and with the internal services of the hospital is the analyzed request. This coordination relates to the substitution of the patient's parent in the human resources of the UMC department, on the one hand, and lack of operational skills between the different actors in the healthcare chain on the other hand (Caroly, 2010; Caroly et al, 2006; Weill-Fassina, 2004). The organization of a reception and sorting unit for patients as well as the coordination between the UMC and the internal services of the hospital according to effective groups were recommended.
